If you have any questions about using Final Forms, refer to the Parent Registration Guide. Even if your student’s current physical paperwork does not expire until after the new school year begins, or if you completed paperwork for a fall sport, please register electronically on Final Forms.
We are asking that ALL student-athletes (grades 7-12) use FinalForms. All forms are to be completed by July 30, 2022 if your child is planning on playing or trying out for a Worthington Cardinal Fall Sport’s Team. Once your student’s physical form is received by the athletic department, we will input the expiration date into FinalForms and then you will receive automatic notifications when your student’s forms need to be updated. The physical form must be completed by the doctor and by law we are required to have a signed copy on file. If you still do not see the FinalForms email, please contact Janene Dunn in our Registration office at 93 or .us. The ONLY form that you are still required to actually print, complete, and file in the Athletic Department is the actual OHSAA Physical form. All you need to do is log back into your Final Forms account and complete the registration forms.) What if I don’t receive an email If you do not receive an email, check your spam folder. The Worthington Athletic Departments have partnered with FinalForms, an on-line program which will enable you and your student to complete and file MOST athlete forms electronically.
